Effective October 19, 2026. Health information custodian: the Nurse Practitioner owner of PMP Healthcare Corp., College of Nurses of Ontario Reg. No. (on request and on every receipt), on behalf of PMP Healthcare Corp., Sarnia, Ontario.
Who we are
PMP Healthcare Corp. provides Nurse Practitioner services in Ontario: virtual appointments, visits to nursing homes and retirement residences, and geriatric consultation. We collect, use and disclose personal health information under Ontario's Personal Health Information Protection Act, 2004 (PHIPA).
What we collect
- Identity and contact details: name, date of birth, address, phone, email, health card number where relevant, and the name of your substitute decision-maker.
- Health information you give us or that we create: history, assessments, notes, test results, prescriptions, documents you share.
- Information from others involved in your care, with your consent or as PHIPA permits: your family physician, pharmacy, the home you live in, hospitals.
- Technical information when you use our website or video visits (for example, that a connection was made and when). We do not record video visits.
Why we collect it
To assess, treat and follow up your care; to communicate with you and, with your consent, your other providers and your pharmacy; to issue receipts for your insurer; to meet our legal and professional obligations; and to manage our practice.
Consent
We rely on your implied consent to share information within your circle of care (your doctor, pharmacist, the care home). For anything outside it, we ask for your express consent. You may withdraw or limit consent at any time by telling us, subject to legal limits. Email and text are not fully secure; we use them for scheduling and general messages only, and for clinical details only if you ask us to and understand the risk.
How we protect it
- Records are kept in an encrypted electronic system hosted in Canada, with two-factor sign-in and an audit log of every access.
- Prescriptions are sent to pharmacies by secure fax from a dedicated line.
- Video visits use encrypted connections; patients wait in a lobby until admitted.
- Paper is kept to a minimum and shredded when scanned.
- Staff and contractors, if any, sign confidentiality agreements and receive privacy training.
How long we keep it
Clinical records are retained for at least 10 years after your last visit (or 10 years after your 18th birthday, if longer), as required by the College of Nurses of Ontario, then securely destroyed.
Your rights
You can ask to see or get a copy of your record, ask us to correct it, ask who has accessed it, and make a complaint. We respond within 30 days. Reasonable copying fees may apply.
Breaches
If your information is lost, stolen or accessed without authority, we will tell you at the first reasonable opportunity, as PHIPA requires, and report to the Information and Privacy Commissioner of Ontario where required.
