Senior care

Built on ten years in long-term care.

Most of our work is with older adults, their families, and the homes they live in. This is where we started.

Nursing homes & retirement residences

An NP on your floor, every week.

On-site coverage for long-term care and retirement homes in Sarnia–Lambton. We chart in your system, write orders within NP scope, and work with your Medical Director and attending physicians. Before any visit we check the home's own policies and coordinate with the resident's current primary care provider so care stays joined up.

  • Acute change in condition, fewer ED transfers
  • Quarterly medication reviews and deprescribing
  • Responsive behaviours, falls, wounds, pain
  • Palliative support and family meetings
Geriatric consultation

One assessment. The whole picture.

For older adults with many conditions, memory change, repeated falls, low mood or a long medication list. In person in Sarnia–Lambton or by video anywhere in Ontario.

  • Comprehensive geriatric assessment (CGA)
  • MoCA / MMSE and dementia work-up
  • Beers and STOPP/START medication review
  • Written report for family and primary care
Medication review & deprescribing

Fewer pills. Better days.

A full profile review with a deprescribing plan you can take to your pharmacist and family doctor. Dizziness, falls, confusion and fatigue often start on the medication list.

Families & caregivers

Book for your mom. We'll take it from there.

Caring for an aging parent from across town or across the country? Book on their behalf. We confirm consent, assess, loop in their doctor or the home, and send you the plan in plain language.

1 in 4 Ontarians were projected to be without a family doctor by 2026, about 4.4 million people. Ontario College of Family Physicians projection, November 2023.
Why a Nurse Practitioner

Same clinical authority. Shorter wait.

Nurse Practitioners are regulated by the College of Nurses of Ontario and can manage most of what a family practice sees. If something is outside NP scope, we refer rather than guess.

How it works

Three steps. No referral.

1

Request a time

Use the form or email the clinic. Tell us who the visit is for and what's going on. Takes two minutes.

2

We confirm within one business day

You get the time, the fee, a video link, and a short list of what to have ready: medication list, health card, recent results.

3

Visit, then a written plan

Prescriptions and requisitions sent the same day. Summary to you and, with consent, your family doctor or the home.

Pricing

Clear fees. Receipts for your plan.

NP services are not covered by OHIP. Most extended health plans reimburse Nurse Practitioner visits; every receipt carries the registration number your insurer asks for.

Your Nurse Practitioner

The same Nurse Practitioner. Every time.

Your Nurse Practitioner is registered with the College of Nurses of Ontario, in nursing since 2016 and in NP practice since 2025. Long-term care is home ground: prescribing and clinical leadership roles, and primary care for residents with complex, overlapping conditions.

The practice comes down to three things: managing chronic disease well, getting the medication list right, and stopping medications that no longer help. PMP Healthcare was built to bring that standard of care to people at home, online, and in the homes where they live.

Questions

Before you book.

Is this covered by OHIP?

No. Independent Nurse Practitioner care in Ontario is private-pay. Most workplace health plans reimburse NP services under paramedical or "nurse practitioner" benefits. Every visit comes with a receipt showing the NP's registration number. Send us your plan name and we'll tell you what we know.

Do I need a family doctor or a referral?

No. Book directly. If you have a family doctor, we send them a summary with your permission so nothing falls between the cracks.

What can't you treat virtually?

Emergencies: chest pain, stroke symptoms, severe shortness of breath, serious injury. Anything that needs a hands-on exam the same day. Controlled substances on a first visit. We'll tell you straight away if your concern needs to be seen in person.

Can a Nurse Practitioner prescribe?

Yes. NPs in Ontario prescribe medications, including controlled substances where the NP has completed the required education, order labs and imaging, and refer to specialists. A few things remain outside NP scope and we'll tell you when a physician is needed.

Can I book for a parent in a retirement home or nursing home?

Yes. Book a virtual visit or in-person assessment on their behalf. If they can consent, we confirm it with them at the start. If you're the substitute decision-maker, tell us. For on-site visits we first check the facility's own policies on outside clinicians and contact your parent's current primary care provider, so the home, their doctor and we are all working from the same plan.

I need a referral to a specific specialist. Can you do that?

Yes. Tell us what you need and why. We review your history, assess you, and refer to the appropriate specialist with the supporting notes and requisitions. Specialists set their own wait times and some require specific work-up first; we'll tell you what to expect.

How does a facility visiting agreement work?

We agree a weekly block, a resident list, and how to reach us between visits. We document in your chart, write orders within NP scope, and work with your Medical Director and attending physicians. Invoiced monthly. No minimum term for the first 90 days.

What do I need for a video visit?

A phone, tablet or computer with a camera, a reasonable internet connection, and a private spot. We send a link by email. If video fails, we switch to phone.

Where are you?

Sarnia, Ontario. Virtual visits province-wide. In-person visits in Sarnia–Lambton, with travel to neighbouring areas by arrangement.
